Creating appropriate address locator for geocoding with US data

I am relatively new to ArcGIS, so these may be simple questions. I am trying to geocode US hospital addresses. In my hospital data file, I have the street number, street name, city, state, and zip code.

I know that I need to create an address locator in order to geocode properly. Should I use the address locator style "US Address--Street" given the information I have in my hospital file?

Also, what should I use as the reference for the address locator? Do I need to add a shapefile containing all US streets?
