arcgis categories thematic map

I want to create a thematic map in arcmap using the fields NAME and NOTES for 3 categories of polygons:

1. where the name cell is null
2. where the name cell is filled
3. where the name cell is null, but it has the note "NUME EBRAICA", which means "name in hebrew"; this note appears only to the polygons where the name cell is empty.

Any suggestions, please? Everything I've tried didn't work.. Thank you!
